For me, it was a karate kick gone wrong. I twisted the foot I was standing on when I was trying to do the kick. That happened when I was 8 years old.

Some people just drop a very heavy item in the middle of their foot

What a strange question.
My mum broke her ankle by falling into a loose drain in the road, the sound was awful.
...and my friend broke his on one of those round-a-bout things you get in kids play-parks, got his foot stuck under it and it dragged him around for a while...not a nice sight.

i guess if you fall out of a tree you'd probably break something.
On an episode of "Monk", "Mr. Monk goes to the Circus", the killer purposely has an elephant stomp on her foot (and break it) so the police would think she wasn't the killer!!
i could never do that!
