It's said that the police van is a fake, and that she was possibly rescued. Check out Sparknotes, they have a lot of information on The Handmaid's Tale.

The ending is open on purpose. But if you read the epilogue (the records from the "history convention", you know that she at least had the opportunity to record her story on tape, so the police van almost must have been fake - otherwise she would never have had a chance to do that.

The theory is that she was on the underground railroad for a while, but no one knows if she made it it Canada or not, and what happened to her.

Personally, I don't think she did. :( If she had, all records wouldn't have stopped so abruptly...

Personally, I like to imagine a happy ending for her. Yes, record-wise she fell off the planet, but if I had been her, I would have taken on a whole new identity and just started completely fresh. Thus the vanishing act. I also like to believe she got her daughter back, but that might be asking too much.
