my daughter has 2 fill out an evaluation form for her assignment (yr5). How does she do this? whats involved?

usually the evaluation form will have some set questions, but if not they generally want to know things like:
-how difficult was it to find information/explain
-did you enjoy learning about this topic, why/why not
-if you worked with a partner what were some of the positive and negatives
- what improvements would you like to make for your next assignment
- how can the teacher be of more assistance
