I actually have a friend grappling with that question right now and was thinking of posting it. I think it depends. Kinda like George said on Grey's-why should you confess to them so you feel better...In a way that is just as selfish as cheating.

If you're going to leave your spouse, yes. If not and it was only a one time thing you shouldn't tell them. It seems like that would be selfish (just to make yourself feel better and ease your conscience) meanwhile, it just hurts them.

No. What they don't know can't hurt them. Once that line is crossed theres no going back. Unless you're caught. Then might as well come clean :)
