wife wants to take child out of state she's black mailing me in an instance so if i sign this document can she hold it against me

No. The document only demonstrates that, at that time and place, you agreed to what it said. You could change your mind two minutes later and revoke the permission.

If you did change your mind, you would need to serve notice somehow to the holder of the notarized document of its revocation. Folding the revocation up and stuffing it into a safe deposit box would not impress a court as well as mailing it to her by certified mail w/return receipt. Or handing it to her in person with a witness.

So My answer is No. No document that you sign relative to your child is absolutely binding. All can be declared void by a court acting in the child's best interest.

If it is really blackmail. you can report her. To answer your question, the court can overide that document if there is good reason.
