your stomach churns food much like the way you could squish something around in a plastic bag. when air is trapped in your stomach that churning motion continues and causes sound (growling)

You are digesting air when your stomach is empty. Hence the growling sounds.

Your stomach has no food in it, only air from the spit you're swallowing.

The sides are churning, sort of gulping for food, but there is no food.

The sound you hear is the stomach trying to digest air.
