In the 2006 film remake of the 1974 original "Black Christmas", Kelli witnesses Kyle, her boyfriend's eyes being ripped out and eaten. She doesn't instigate it but really just looks on in horror!

Then of course, there is the old classic made around 1950, "Samson & Delilah" starring Hedy Lamarr as a very sexy Delilah, who doesn't witness Samson's eyes being gouged out by the Philistines, but I guess you could say instigates it, as she cuts his hair off which weakens him and which leads to the Philistines overpowering him.

In the 1968 British-made "Oedipus The King", Oedipus (Christoper Plummer) blinds himself with his mother Jocasta's brooches (played by Lilli Plamer) when he discovers that she has hanged herself. So her suicide instigated his blinding of himself, not surprising when your mother discovers she has unwittingly been sleeping with her son!

Not sure but didn't some female promted eye pulling out occur in the 1970's horror movie "Ruby".
