Interesting question, and one that has been the subjject of several studies.
The brief answer seems to be "self consistency."
This means that once they've done or said something one way, even if it's partly or completely wrong, people would rather do it over the same way because correcting themselves would be an outright admission that they were wrong, at fault, mistaken - however you want to word it.
In other words, "I see myself as being a nice, quite intelligent sort of person. I'm not going to blow that view of myself by admitting that I made a boo-boo. That would be inconsistent with my self image. That's not who I am."
The only way people can get round this, that I know of, is to recognise that who they "are" and their behaviour are two different things. I can be a nice, quite intelligent sort of person AND I can sometimes make mistakes. It's ALL behaviour - NOT what I am. And behaviour can be changed, without changing who I am.
If I accept this version of things I wouldn't feel undermined or disloyal to myself if I admit to making a mistake from time to time. And if I admit my mistakes I can learn from them and do better in future. If I won't admit I've made a mistake it's very unlikely that I'll learn from it, and very likely that I'll make the same mistake again, sooner or later.

some people are hard headed and just don't learn from his or her mistakes. they think what he or she does it right no matter how big of a mistake it is, and they are just plan out ignorant to everything around them. they don't see it as a mistake because they choose not too.

Memories are stored in clusters of neurons. They become linked and habits are formed. It is hard to break the clusters and fragments of thoughts and memories of events.
