"what's up" is similar to "how are you" or "how are you doing". respond the same way as you respond to these 'greetings' (e.g. fine, good, very well, etc.)

Usually, when someone asks me "What's up?" I usually respond, "Nothing really. How are you?" Just act casual, don't be like "Oh, my god! I love you!" - (Don't come off too strong)
