This is an excellent question for your teacher, not for a forum. Ask for the rubric, which is the list of rules and guidline for preparation.

A basic folio looks like this; bind together the following in a nicely designed binder or folder:
1. Educational Goal Sheet - a paragraph on its own sheet listing what you intend to do and learn in the class.
2. Samples of your work, filed either by date of completion, showing competency for certain goals met, or showing bad to good (the progression of learning).
3. End with a reflection - a one page summary of whether you met all of your goals, how you did it (aka - what you learned) and fun or interesting things you learned along the way.

**Some teachers ask for a reflection sheet on each assignment, and others ask you to include ALL of your assignments. This is why I suggest you ask the teacher.

Your teacher probably has the portfolio guidelines in you class's course syllabus. I have to do one for my English Composition class, but she provided a table of contents for us to put in our Portfolio. Basically in my portfolio she wants all the work we have done for the year organized in sections like class notes, free writing, journals, Goal sheet, Strengths and weaknesses chart, in class exercises, and extra credit: she will look at all this stuff and give us some kind of grade depending upon how organized and neat we were. Not all Portfolios are the same so your class portfolio requirements may be different.
