I do the finger tapping thing often enough, for me I like to try to come up with different rhythms and off beat sounds, I like to make things up and sometimes I just do it because I'm bored. Other times when I do it I think it might just be habit, I like to keep my hands busy and have an oral fixation as well, I am a smoker attempting to quit and found the finger tapping has increased along with chewing on things. Sure some people might do it cause of their nerves but that is not the case for me, I am rarely nervous or anxious.

Depends on the situation, but it usually means I'm bored, and I can't wait for this to end.
It's also very rude if you're on the receiving end.
Call them on it. Ask, "Am I boring you?" or "Is there somewhere else you'd rather be?" A few of those, and they'll stop it if they're smart.
