Is there a certain time limit you have to turn in the medical certificate before you get approved for FMLA or can you get approved before you turn it in?

You can indeed get approved ahead of time (an example is major surgery that is scheduled ahead of time). However, you still must fill out the paper work to be approved. You cannot be approved until you turn that paperwork in. BUT, that can be filled out AHEAD OF TIME for planned surgeries, etc. As for a medical certificate, your physician will fill that out. The certificate itself is NOT required by FMLA, but many employers require that specific form to be filled out. A letter from your physician will, legally, suffice in place of such a certificate.
