Take the time to find out about the things he loves. For example, if he enjoys golf, look for a special golf item he might not buy for himself. Does he have a favorite color? You can use that info to buy all sorts of things in that color to personalize your gift. Does he like cooking? Go to cooking/kitchen stores and websites and find things that are special items most people won't usually buy for themselves. I find looking through stores brings lots of items I didn't even know existed to my gift list.
How personal the gift is can be determined by how long and how intimately you know him. Satin boxers for a finance, a book for the boss.
If he has his own house, maybe a gardening book; if he has a computer, how about a book about some software he could use? Hope this gives you enough to start on.
