Most evidence points towards the Neanderthals, though they were not Homo Sapiens. Cro-Mags were probably first (closely related to) humans to do that.

Probably the first people that wanted to dispose of the bodies without predators and scavengers getting at them. That would be quite a while ago.

In terms of formal burials with some ritual or ceremony, the oldest discovered grave site is I believe from the neo-lithic period.
