When one of my Bettas got ich I found that turning the water temp to 82 for 12 days and adding melafix every day cleared up and it has not come back since.

If your loach seems to be the worst one and showing no signs of improvement you may want to quarantine the little guy and treat him more aggressively.
Just stick him in a bowl or tank by himself and medicate him. Stress is a common source of ich (or rather the fish's natural defenses become reduced when they are stressed and become prone to ich)
