This site has lots of info and pics to help you diagnose what's wrong. It has helped me a lot recently.

Good luck with your betta! I hope this helps you figure out what it is and how to treat it.

Oh yeah, to the answerer who automatically said it is ich, ich is characterized by white spots on the body, not necessarily scales falling off. Research a little please.

Sounds like the fish has fin rot or ick. You can usually get treatment from a pet store to put in the water for these conditions..however , I know betas are seen in bowls all the time but it isn't an ideal environment. They should be kept in a filtered tank Putting the lamp over the bowl may not be the best thing either. If the bowl gets alot of natural light then that should be enough. Good luck!

Don't use bottled water, there are mineral in tap water that can help your fish. Go to your local pet store and get a Ph balancer to remove the toxic chemicals (i.e. chlorine, fluoride) from the water. There are also several medications available to help with fin rot. If you go to a petsmart or petco just ask and they can point you in the right direction.

Scales falling off... Are they getting shorter with a red colour at the edges?
If so, it is fin rot... It could hapen due to dirty water, stress...
go to www.bettatalk.com and check in the betta health >> diseases category... they have pictures showing you what fin rot looks like and how it can be treated.

Did you overfeed him? Betta fish can only be fed 3 little tablets a day, or they will DIE.
It could also be the light, maybe you should dim it a little. Not too dark.

Are you treating the water EXACTLY according to directions? My wife thought that you could cheat a little bit here and there if you were short on chemicals. Each time it died...

he has ICH.....get an ICH treatment for him or he'll die.....also do not scrub the tank clean....that was our mistake. They need the bacteria.
