When deciding to be called Americans, the deciders all had the notion, that the United States would controll all of the Americans any way, so it didn't make much of a differance.
Plus, United Statesmen didn't have a good ring to it.

In the Civil war you had the Confederate States of America vs the United States of America and naturally the were labeled teh Conferderates and the Union, respectively. If there was another civil war with a hypothetical country call the United Provinces of American, they would probably be call the Staters vs the Provincers.

America was America before South America was so called. We were first. Before Canada or South America. Even if Canada had a civil war, they would still be called Canada.
