Perhaps you can find the situation that fits best for you at this site. You will have to read through the letters and answers though . There is also an old rule that a man's first name is not to be separated from his last name so if you are using Mrs. and Mr. and the woman took his last name in marriage, it would mean you should put hers first as in Donna and Tim Smith.

This is not about grammar, it is about etiquette:
On the envelope you write: Mr and Mrs Tim Smith.
In the letter, you start by: Dear Donna and Tim.
So the man first on the envelope
The woman first on the letter.

it honestly doesnt matter but i do see enveloes that start with the mans initial
like : Donna and Tim Smith are the couple
on envelope : T. and Donna Smith

Mostly, the man's name should come first. However, in some cases, the woman's name may come first. For instance, if it is a business invitation and you are inviting a married couple and the woman is the President and the husband is the Vice-President of the company, the woman's name should come first.

If you are addressing a letter to two married people you should address it as
"Mr. and Mrs. Tim Smith".
If there are more then three people that you are addressing it to you can also use
"Mr. Tim Smith and Family" formally
or
"The Smith Family" informally.

On a casual letter, it doesn't really matter, but normally the man's name comes first. It's the only thing "women's lib" left them with. I guess it's OK.
