All home improvment store sell concrete patch in cans or caulk type tubes. It is not sandable but it can be ground with a grinding wheel if needed.

Sika makes a great caulk that will fill the cracks up to 3/8 " wide.
Larger cracks require a backer rod filler before filling them in.

you have use a concrete glue before you can apply anything just apply with a paint brush or the concrete will fall off a good product is quikcrete it is availible anywhere
