I'm sure there's urination holes that will allow you to pee, and I beleive the back would be open, so #2 isn't really a concern. You'd probably have to do a little bit of wiping after peeing though.

Why would you want to wear one? That seems really unsual to me. Seems to me that if you want to abstain from sex, you should just not let yourself get into a situation where it could be an option. I don't think a chastity belt is the right course of action. But then again, that is just me...
