I tried entering the mailto html code into the page, but FrontPage webots eliminate it when I save the .asp page. Any clues on how I can get an email and have it write to an Access DB?

You can save it into a database sort-of-like file, but in order for it to be emailed to you, you need to have a server-side application which goes beyond the use of FrontPage. Usually your hosting service or other company provides you with this (or you can learn how to program them yourself by learning CGI). To save it into a database, go to the page where you want the form and click Insert > Form > Form. When you have put all the fields in place, right click anywhere inside an empty part of the form and click on "Form Properties," there you can choose to save the results on a database, a CSV file, or any other script you wish.
