From Babycenter:
A diaper rash left untreated can easily become infected with yeast — even if your child is a boy. Babies taking antibiotics or breastfed babies whose mothers are on antibiotics are more susceptible to yeast infections. That's because antibiotics kill the good bacteria in the body that keep yeast in check. Without these bacteria around, yeast can grow more abundantly.

You can usually tell a yeast infection in the diaper area (versus a plain old diaper rash) because the rash may have multiple red bumps, pus-filled bumps, or a scaling pattern on the infected skin. In addition, the rash may extend along the pubic area and onto the lower part of the abdomen. Another big clue: A yeast rash tends to hang around for more than two days and doesn't respond to any traditional diaper rash treatments. It also usually shows up in the skin folds of the groin area.

The best treatment for a yeast infection is a topical anti-yeast or antifungal cream such as Lotrimin AF, Nystatin, or Monistat. These are available without a prescription in the drugstore. Use it as directed on the label. The rash will take a few days to clear up.

My little girl is just getting over one...it's been a long battle, we've been treating it for over a week and been to the doctor twice. Good luck :)

Yeast diaper rash is caused by a type of yeast called candida, which also causes thrush in the mouth. The rash looks different from a regular diaper rash. It is very red, and there are usually small red bumps on the outer edges of the rash. This rash requires treatment with medication.

Yeast rashes can happen even if you're careful about keeping the baby's diaper changed in a timely manner, especially if the mother is breast feeding and on antibiotics, or the baby is on antibiotics. Newborns can aquire it during birth through the birth canal. The best thing to do about a yeast infection is see the pediatrician. They can give you the proper anti-fungal medication, and it's good to try to let the baby 'air out' without a diaper on when you can. Over the counter diaper creams will NOT cure this.

Because diapers are a great wet place for bacteria to grow, babies are a bit more prone to infection. Dry and keep baby with a nice dry diaper, some desitin and baby powder or corn starch will help as well. If you have tried these methods and still continues to be a problem you may want to consult with pediatrician. And no it's not serious unless left untreated.
