I hosted a leadership conference for youth and I need an evaluation form for them to tell me what they thought of the conference.

It is very hard for someone to write an evaluation form without having attended the conference. However, it will be easy for you to do it. Brainstorm a list of all the activities you presented or hosted. Then make another list of the objectives or what you were hoping the attendees got out of the conference. Next look over your list and generate questions that will give useful feedback (for example: how you can improve the conference in the future).

Remember, people won´t want to spend much time filling out evaluation forms so keep it simple. (Example: Did you find activity X useful/helpful?) or you could list the activities and have them rate them from 1-5 , 1= poor - 5= excellent.

Finally, before you hand out your evaluation, test it out on a friend to see if they understand what you are asking.

The activity presented me with new information (scale of 1-5)
