If you don't have a plot, what do you have planned out?
Why not begin by working on your characters. Free write about each of them for a few pages, and get to know what kind of people they are. Once you delve into their likes and dislikes, natural conflict is going to emerge from their personality differences!
I can't give you a plot for two reasons. First, those are my ideas and if I want those books written then I will write them myself. Ideas may be a dime a dozen, but I still am not giving up the ones I have! Second, if you aren't writing about a subject you are passionate about, your writing will be flat and you will probably never finish the book. A novel has to come from within you!
Don't worry about a title until you finish writing the book (or get close, at least). Generally these things appear from the story.
Happy writing!

Do not start with a title. That should be the very last thing you do with your book, because more than likely you will end using a line from the book. If you haven't written any lines, then you can't come up with a title.
As to how to approach a book, start with two thoughts: First, what is it you are trying to say? Are you writing something new and different about love, about human relationships, about revenge, about what? Stick with a comment on the human condition, what humans are, their strengths and weaknesses and you will be okay. Secondly, think of a conflict. Who are your characters and what are they going to be fighting over or struggling with? Conflict is far more important than plot.
Okay, and finally, write about a place you know about. King writes about Maine, Steinbeck about the Salinas Valley, Rice about New Orelans ... why? Because these are places they live. Most authors think the place they live is 'boring' because that is what they know and where they have always lived. However, for people who live in different places, the place you live is fascinating to them, if you describe it well.
Finally, when you describe your characters, make sure they are believeable characters, even if they have their own little weirdnesses. People want to be ble to identify with characters, but they also want to read about people who are different.
Average people in average places with average issues make for an average story ... find the believable but the odd. It will be fun reading.

If you don't have a plot, what's the point of the rest you say you have planned out? The average annual earnings for all the authors is just over £4k? Are you sure you want to head down that road? (Of course, if you make it into the 10% you may well earn £400 million - but I do think you need a plot!)

Let the plot come naturally. Just keep jotting ideas and the story will evolve on it's own. When I wrote my first book the original plot was completely different to what it was going to be. Just keep on jotting down ideas and little pieces you want to use in your story, like lines etc, and eventually the plot will come but don't force it. When I write my books the story writes itself. It quite often take sturns that I wasn't expecting and i find myself dropping some points because they will no longer fit, however much i want to use that point. But that will then lead to the next book using that story.

There is little else to plan out!
If you want to write something, I wish the best of luck to you. However, I think for a plot that you will find easiest to write, relax and give your imagination time to work. Long car journies where I have nothing to do twig it for me.

Why write a book if you do not even have a plot? Leave it to the pros or people with an imagination.

the title doesn't have to be the first thing you work on any characters yet i find you can often build a plot around them once you get to know a little about them try writing character profiles first get to know what drives your characters

Why not just get everyone else to write it for you, too?
