All you need to do is change the last name on the card. Take your driver's license or state ID, marriage certificate and current social security card to the Social Security office and have them help you fill out for a new card. Some states require you to have your birth certificate so you might want to take that along as well. Your married name must be on your social security card before you can file taxes jointly.

No, that number is yours for the rest of your life. You MIGHT (or might not) want to change your last name on it but you don't get a new number. Most people don't change anything.
