Whales breathe air like us. The respiratory system, however, is very complex as compared to humans or other mammals.

Blowholes: Whales have blow holes on top of their heads or end of snouts depending on the species (2 holes like nostrils without projection). They exhale CO2 (this is when the fountain of water comes out) and then inhale Oxygen.

Trachea: The blowholes are connected to the lungs through this windpipe or trachea.

Lungs: This is where the oxygen-CO2 transfer happens.

Whales have lungs just like all mammals. When they surface, they use their "blow-hole" to exhale, then they inhale more air, and in essence hold their breath while under water. However, they can hold their breath a lot longer than us!!
