the +++ is the amount of protein leaking out of her kidneys, this level will cause permanent damage to the kidneys if it is not treated, ive just had a baby and had pre-eclampsia I started at ++ then got to ++++ before I was given a c-section at 35 weeks, it is a serious condition as it leads to eclampsia which can be fatal so the doctors will be monitoring her very carefully, if they cannot control her blood pressure with medication the only remedy is to deliver the baby, she will improve very quickly once delivered, my blood pressure stabilised after 4 weeks, if she has damaged her kidneys she may have to take some medication, my kidneys are scarred due to renal failure at an early age and my last pregnancy did damage them a bit more, it just means I take a bit more medication for my blood pressure and get monitored by the hospital every few months, hope everything goes well

It would have been easier to answer your question if it were in proper form, but I take it your wife or friend had pre-ecclampsia with her first pregnancy and now has proteinuria that may or may not be pre-eclampsia again? The +++ is not a very accurate thing. Were it really important in the decision-making process, she might have had a 24-hour urine collection for protein to see if she's spilling, say, 100 mg of protein per day, or 4000. As it is, the spot check on the urine shows +++ on a scale of 0 to ++++. Pre-ecclampsia is largely seen in first pregnancies, and if other signs are absent it may be that the nephrologist wants to check for things like glomerulonephritis sometime after delivery. This is all a wild guess, but maybe it'll help you form things in your mind.

3+ of protein is quite a lot in pregnancy, it means that her kidneys are having trouble coping. If she has pre-eclampsia she will likely also have high blood pressure, swelling in hands and feet, persistant headaches and sometimes spots before the eyes.
They can treat pre-eclampsia to a point with total bed rest and tablets to reduce the blood pressure, but if the BP/Protein does not come back under control then they may need to deliver early, they will probably give steroid injections if the baby is below week 37 to get the baby's lungs ready, but the baby can survive from surprisingly early in the pregnancy, my little sister was born at 27 weeks and was only in hospital for a few days!
Generally the symptoms of pre-eclampsia disapear as soon as the baby is delivered, seeing a kidney specialis may just be precautionary as pre-eclampsia is more prevalent in people with existing kidney problems, but it doesn't necessarily mean that there is anything at all wrong with her. Being dehydrated, eating lots of sugary foods shortly before a water test and water infections can all present protein in the wee. . .

Pre-eclampsia can sometimes happen for no reason but sometimes it is due to an underlying kidney problem. I think its routine to see the renal specialist after suffering with pre-eclampsia because they just like to check it all out and find out how the kidneys are functioning, etc. Once the baby is born there is no more pre-eclampsia and everything should slowly go back to normal. you could be talking about a condition called pre eclampsia. it is caused by high blood pressure and protein in the urine. difficult to say if it is a problem related to pregancy or she has some kidney problem . your question must be made much more clear.
