You MUST go to a lingerie shop. I never got fitted and was wearing the wrong size for way too long. You won't believe the difference the right bra can make in your appearance.

you are wearing the incorrect size. if you are falling out the front, the cup size is too small. If they are riding up your back, i believe that means your band size is too big ( think, or it means it is too small).
go to a store like nordstroms or macys or something and have a pro measure you. I actually do not recommend going to Victoria's Secrets to get measured. Nordstroms has the best service when it comes to lingerie. They will tell you waht fits, what doesn't and help you find the best bra for you. Oprah's show did a whole episode on it.
The weird thing with bras though, is that even if you get measured, depending on the brand, you may or may not wear the same size. I am a 38C in like 90% of my bras, but in one brand I am a 38D. You have to try a lot of them on. And you will find that you like some brands better than others.
If you are big busted, i like brands like felina, dkny, wacoal, natori, le mystere (super expensive but worth it), victoria's secrets body by victoria ipex.

You probably are getting the wrong size. Measure your chest just under breasts and add 4-this is the number part.
Then measure over th biggest part of breasts-for every inch difference add a cup size-1=A, 2=B, 3=C etc.
Imagine my surprise to find out I'm a 44 DDD!
Good luck.

Go to a shop that customizes bras. Get measured and go from there. It sounds like you are getting bras that are too small in the cups and too big in the band.

I like to go to a place that will measure you first. That way you will get the perfect fit the first time.
