Its a "Bill of Lading" or a BOL.

Basically this is a document you sign stating what it is you are shipping.

The shipping company never should have left your premises with the package until you provided this.

I've only seen this with larger shipments. FedEx and UPS packages don't need a BOL.

It is shipper that list the contents of items. pretty much required on all packages now.
