First off, everyone is over (and some under) reacting about this subject. The results obtained while using a Ouija board depend greatly on the seriousness of the person using it. If you're just BSing with it, nothing of significance will happen. If you are serious and truly focusing, then many "strange" things can occur. As a rule of thumb, I must insist that if you want a Ouija board for SERIOUS use, that you either have a higher-than-typical ESP level, or train yourself to read the types of vibes a person's spiritual energy gives off. Also, if you should happen to make contact with a spirit, don't make ANY type of deal or do or ask anything that could hint even in the slightest as a sign of weakness. "Evil" spirits will very quickly take advantage of this.
As for making one-
If it is for play, then you can simply draw one up on a large piece of paper and use anything that slides easily that you can see through (be it plastic or anything transparent, or something with a hole in it)
If it is for serious use, find or create your own design and engrave or burn it into a piece of solid wood and have it finished. As for the guide, I'll let you come up with your creative ideas, because the possibilities are truly endless. Before use, have it blessed. As a Solitary Witch, I blessed mine myself, but if you are not, you could always find some one who does practice Witchcraft, Divinitory Arts, or a Priest or Priestess.
In all, have fun and be safe. If for any reason you begin feeling uncomfortable during the use of your board, end the seance in proper order and close up shop. Take care sweety!

It should be a simple matter to ink or burn the alphabet and yes and no onto a wooden board (sand the edges to avoid splinters). If you don't trust your own calligraphic ability you can use a stencil set from an art store (not too expensive). You can either make a triangular wooden pointer or a reading magnifying lens for choosing the letters.
I wouldn't really recommend it. To have any real effect it relies on the user's ability to convince him or herself that the effects are real. So most of the time you end up either freaked out or bored. But hey, maybe it'll be fun for you. Good luck.

Don't use ouija boards. Sylvia Browne - the psychic - even warned against them. When my daughter was in her teens, she was with a group of friends, and they played it. She was too scared to even tell me what happened, but she said they threw the board out in the garbage and she'll never go near one again. There's something about EVIL spirits connecting with you disguised as friendly spirits. I wouldn't play with one.

Ouija boards are very harmful and can cause evil to enter you home and do harm..this is not a joke, and at this time when the world is kind of mixed up, and we have war, it is extremely dangerous. Please stay away !! YOur life may depend on it.

i just have one thing to say, even though i dont know anything about MAKING a ouija board i say DONT USE THESE, they are bad, and if anyone else has played one im sure they will agree.

I'm sure you can find a schematic or at least a picture online, if you just Google under "images".
Ouija boards are harmless fun (or boredom), everyone.
[added] I'm clearly one of the "under-reactors" that Shel K is talking about, but her answer is fun!
