As far as The Bible is concerned, how it was created wasn't explained but it definitely exist because it was mentioned in the Bible 14 times. Hell is also known as the lake of fire or "second death" (Revelation 20:14).
The lake of fire is the ultimate destination of everything wicked--Satan, the beast, the false prophet, demons, death, Hades, and all those whose names are not recorded in the book of life because they did not place their faith in Jesus Christ.
In St. John's vision in the book of Revelation, it does not permit any gray areas in God's judgement. If by faith, we have not been identified with Christ, confessing Him as our Lord, there will be no hope, no second chance, no other appeal.

If I remember correctly, Hell has not yet been created, but when the war between the forces of God and Satan is over earth will become hell for Satan and his sinners while the believers will be taken up to New Jerusalem.

The problem with the word hell has its roots in your question - history. In the KJV Bible the word is usually translated from Sheol (Heb.) and Hades. (Gk.)
So many religions and philosophies have defined it. Anyone familiar with today's TV programs of Zena and Hercules have made most of us a little familiar with Hades!
Here is a quote from Wikipedia regarding Hades, " Further information: Hades The Greek underworld is a general term used to describe the various realms of Greek mythology which were believed to lie beneath the earth or beyond the horizon.
These include: 1. The great pit of Tartarus, which was originally the exclusive prison of the old Titan gods, but which later came to mean the dungeon home of the damned souls ; 2. The land of the dead ruled by the god Hades . . . " End of quote!
Unfortunately, Greek philosophy has influenced how most Christians view Hell. The Bible itself teaches only that death is a sleep like state for all, wicked, righteous, etc. It does hold out the hope of the resurrection for some but not until the second coming of Christ. see box below.
(AB) Job 14:12, 12And man that has lain down in death shall certainly not rise again till the heaven be dissolved, and they shall not awake from their sleep.

Hell was created when humans figured out that life was a series of choices, and then based their decisions on what are called the seven deadly sins, therefore creating hell for themselves right here on earth.

People who wanted to control people created Hell as a bad place to fear if they didn't conform to their ways. That's why we have church laws, for the men in dresses with a control complex to rule the masses through fear. John created the fire and brimstone factor to keep Christ's followers close to the church and not stray. But Hell is merely a state of mind, and not a real place.

The realisation that we are all capable of doing horrible things, the desire to blame it on something else, and the need to both have a sense that we can't get away with it even if we can get away with it in this life as well as the desire to be punished for our wrongs and to punish other wrong-doers.

God created hell for those that deserve it. It came to exist after some of the bad Angels refused to obey God and thought that they were greater than Him. Therefore God created hell for Satan and the other evil angels (devils) and cast them into hell.

Hell was created when God gave an angel by the name of Lucifer a task. Lucifer did not want to complete this task, and was rejected from Heaven. He figured that if he couldn't be happy, no one would be happy, therefore creating Hell.
