I believe it's suicide because the lyrics also say "Like Romeo and Juliet". Anyone who read Shakespeare's play knew that Romeo and Juliet committed suicide because the circumstances that they lived under, the feuding between the Montagues and the Capulets, would not allow them to have a relationship even though they were in love with each other.

(The song is about suicide) just read the lyrics closly in you will understand why why it means dont fear the reaper in 40,000 men in women every day
All our times have come
Here but now they're gone
Seasons don't fear the reaper
Nor do the wind, the sun, or the rain
We can be like they are
Come on, baby
Don't fear the reaper
Baby, take my hand
Don't fear the reaper
We'll be able to fly
Don't fear the reaper
Baby, I'm your man
Valentine is done
Here but now they're gone
Romeo and Juliet
Are together in eternity
Romeo and Juliet
40,000 men and women every day
Like Romeo and Juliet
40,000 men and women every day
Redefine happiness
Another 40,000 coming every day
We can be like they are
Come on, baby
Don't fear the reaper
Baby, take my hand
Don't fear the reaper
We'll be able to fly
Don't fear the reaper
Baby, I'm your man
Love of two is one
Here but now they're gone
Came the last night of sadness
And it was clear she couldn't go on
Then the door was open and the wind appeared
The candles blew then disappeared
The curtains flew then he appeared
Saying don't be afraid
Come on, baby
And she had no fear
And she ran to him
Then they started to fly
They looked backward and said goodbye
She had become like they are
She had taken his hand
She had become like they are
Come on, baby
Don't fear the reaper

7 years later:
As stated, Dharma was guessing at the number of people who died every day (in 1976). The verses "40k men and women everyday, Like Romeo and Juliet; Are together in eternity" mean that 40k people die daily with hopes of being reunited the afterlife. 40k suicides daily is preposterous.

This is all I could find, I'm afraid...
DON'T FEAR THE REAPER (1976)
By Blue Oyster Cult
All our times have come
Here but now they're gone
Seasons don't fear the reaper
Nor do the wind the sun or the rain (we can be like they are)
Come on baby (don't fear the reaper)
Baby take my hand (don't fear the reaper)
We'll be able to fly (don't fear the reaper)
Baby I'm your man
La-la-la-la-la-la-la la-la-la
Valentine is done
Here but now they're gone
Romeo and Juliet Are together in eternity (Romeo and Juliet)
40,000 men and women everyday (like Romeo and Juliet)
40,000 men and women everyday (redefine happiness)
Another 40,000 coming everyday (we can be like they are)
Come on baby (don't fear the reaper)
Baby take my hand (don't fear the reaper)
We'll be able to fly (don't fear the reaper)
Baby I'm your man
La-la-la-la-la-la-la la-la-la
Love of two is one
Here but now they're gone
Came the last night of sadness
And it was clear that she couldn't go on
Then the door was open and the wind appeared
The candles blew and then disappeared
The curtains flew and then he appeared (saying don't be afraid)
Come on baby (and she had no fear)
And she ran to him (then they started to fly)
They looked backwards and said goodbye (she had become like they are)
She had taken his hand (she had become like they are)
Come on baby (don't fear the reaper)...
Yeah you're right; it doesn't make much sense at all.
Must have been high when they wrote that song eh?
The only thing I can think of is that 40,000 means the amount of people who die everyday but it does sound excessive, don't you think?
The songs tells us not to be afraid of Death (the Reaper) when the time comes.
And the reference to Romeo & Juliet is one of suicide, of course, but not that many people kill themselves every single day, surely?
And what's this about marriage?!?
Could that figure (40,000) mean the amount of people who get married everyday?
Yeah that would make more sense, especially back in the 70's when that song was released, right?
Anyways, I hope you find what you're looking for!
In the meantime, take it easy buddy!

The lyricist is offering an ironic 'excuse' not to fear death; 40,000 men and women *die* every day, or it could be a metaphor. It's a common joke among some married men that marriage is about as bad as death. :)

For the lines that begin, "40,000 men and women," Dharma was guessing at the number of people who died every day.

They die, I would imaging. Though that seems like a small number. Maybe it's the number of suicides.
