An angel food cake. :)
Or you can just let him have a cake and them keep sweets from him till you are ready to give them. 1 cake will not hurt him, but if you feel strongly opposed to it, do some research and see what you can come up with. 1st b-days are meant for cake smearing faces and bodies.

Actually the least sugary cake is an angel food cake and you could use fresh fruit like strawberries as a frosting. I do this and slice my strawberries with a little water and sugar and leave it for about an hour why is makes more juices then pour it over the cake and put some cool whip on that. This is a recipe that done right a diabetic can even eat. Depending on the particular ingredients you use.

Make a cake that replaces the sugar with applesauce. Having no sugar in a cake generally doesn't work, but you can lower the sugar content considerably.

Here's a link to a very low sugar sponge cake. Until my Dad had better control with his insulin, we had to come up with birthday cakes that had hardly any sugar. Our family liked it, but it wasn't the 'most fabulous cake ever'.

How about a fruit salad of some sort? When you say "sweets" do you mean anything sweet or junk food sweets? There are ways of making a cake low fat and a lot healthier. Substituting apple sauce for oil, and egg whites for eggs are some examples. Good luck!

I guess it depends what you consider sweets. You could always make a carrot or banana cake as they may be healthier options to chocolate, but they are really still sweets. What does he currently eat - can you adapt something? Your son won't mind what it is so you do get a lot of freedom to create something out of the ordinary. Good luck

You could try just a regualr pound cake or applesauce cake. This is for his birthday I would do the whole cake icing thing for this day and then go back to regular routine, it won't ruin him

A juice-sweetened carrot cake is what we had for my daughter's first birthday.
Banana cake, or muffins would also be good for a baby.

My son had a tiny yellow cake. But he wouldn't eat it, just picked at it!

Angelfood cake
Carrot cake

Or try searching online for a sugar free cake recipe. Maybe use splenda or something instead of sugar.
