The term explode may be a little dramatic, but nail hammers can have chips break off the sides of the head if they're used to hammer steel or concrete. Ball peen hammer heads are hardened differently than nail hammers and are made for hammering steel and concrete. The old adage about using the proper tool for the job you're doing applies.

Absolutely, hammers as well as any other hardened metal when struck the wrong way with the wrong material have the capability to chip and fragment, sometime explosively so. Never attempt to prove this unless you have a death wish.

If the casting or forging was not properly stress relieved, it could fracture/shatter under use. This might resemble an explosion.
