there is really no way to "Repair" concrete cracks. they can be filled with joint fillers, but that is only a patch. it doesn't cure the fault.

Don't fill with a concrete product as a few suggest. it would only be a temporary fix. movement between the pieces will break it up.

a flexible joint filler is what you want. just ask at a big box store, building supply yard or good hardware store.

if the pieces are shifting you need to stablize then, bigger piece would be drilled and power grouted to resupport them. smaller piece could be lifted so crushed stone can be added below.

If you want to do this yourself, your local hardware store should have some type of crack filler or repair available.
Someone there might also be able to give you more information on how to do it.
It's really not complicated at all.
