A person who is Rh positive can have transfusions from either an Rh positive person or an Rh negative person.

An Rh negative person should only have transfusions from another Rh negative person (if an Rh negative person has never been exposed to Rh positive blood, either through transfusion, pregnancy, etc., their first transfusion of Rh positive blood will not harm them, but they will develop antibodies and become sensitized to it. However, if a sensitized Rh negative person receives Rh positive blood then the result is almost inevitably fatal, therefore all Rh negative blood types (regardless of sensitization) are usually not given Rh positive blood).

For ABO blood types:
O: receive only from other O blood types, donate to anyone

A: receive A or O blood, donate to A or AB blood types

B: receive B or O blood, donate to B or AB blood types

AB: receive any blood type, donate only to other AB blood types

ABO antigens are preformed, so a reaction will occur regardless of prior exposure.

For a blood transfusion to be successful, both Rh factor and ABO blood type must be compatible. Also, though it does not usually cause a problem, it is always best to transfuse the exact same type of blood into a person because there are antibodies in the blood to be transfused (though not very many once dispersed throughout the entire body) that will react with the recipient's blood.
