All depends on how often you go pee. You should change it every 4-6 hours. Yet when you pee some of the pee can get the string wet. That can get annoying very fast.

No you sure don't have to change your tampons every time you pee.

The urine comes out of a different opening than where the tampon is placed. The only thing that may happen when you have to pee is that the string of the tampon that hangs out may get a bit wet from the stream but that's it and the string doesn't stay wet the whole time after you go pee. It will dry out until the next time you have to go.

No. Changing the tampon too often may cause irritation. Follow the instructions in the box for how often to change it.
