No, actually not wearing a bra usually improves back pain. This makes sense, as the body evolved to hold its own breasts without a bra, the back pain is because the bra shifts the parts of your body that bears the weight of the breasts.

I used to never wear a bra, and my grandma would tell my I would sag, and I would look down and say I'm really not worried about it. Now I am in my 50's, my breasts are no bigger, and they don't sag much, probably the same as any woman my age. I don't think it makes a difference. I wear a 36A.

your genetics and weight will be the deciding factors. There is no other way to increase your bust size other than surgery.
But women with large breasts do tend to have back pain, with or without a bra.
