Okay first lets start with the classics:

You really can't go wrong with any of these they are very established names in the industry. You create a magazine spread prfessionally with an adobe photo shop software. I am assumimg you are talking about an ad in the magazine? I believe advertizers call the magazine you would like your fashion spread to be in and pay for the ad to be in the magazine. Also, the company in which the spread is for, for example DKNY, is responsible to have the photos taken.
If your talking about a fashion spread that the magazine wants in their own magazine, they are responsible for the models and photo shoots, and approval to have clothes worn and shown by the designer.
