the word "psychotic" is the key to your question....
Jason kills as a punishment for wrongdoings. Everyone he ever killed did something either sinful in his eyes. You can't count Jigsaw in Saw because he never kills anybody; he just presents the situations to the victims and they mess up the solution themselves. Michael Myers basically kills his family if possible. Sure he gets a few stray people between, but he is mostly after his sister and niece. Chucky is a cold stone killer, but he also choses his victims because they somehow get in his way as he boogies through life. He has an agenda, and those people were in the path as he went toward it. That leaves Freddy...from all the major players. He gets into your dreams, where you cannot control the situation unless you are very good at fixing your nightmares as they happen. Freddy, in case anyone has forgotten, is not the same as the other icons of horror. He is or was a child molester. He first molested kids in the Elm Street neighborhood. The parents, very rightly, killed him and burned him up in the furnace. Then he went after their kids. Now that is unjustified in any way you look at stuff. Freddy didn't want to pay for his crimes. He wanted the parents and their kids to pay for killing him. He feels no remorse for his orginial actions. He is playing the 'poor me, I was born of a nun, trapped in an insane asylum with a thousand maniacs, so that is why I am messed up' card. Boo Hoo for him.
I vote for Freddy as the most cold blooded of the main horror icons.

I think the one really physotic killer is guy who Norman Bates' character was based on, Ed Gein. He made clothes of peoples skin, used their bones to make furniture and household items and he would steal bodies from graves to get the parts. He even dug up his own mother and wore her skin to be near her. Sickko!!

I'm going with John Doe from SE7EN. The methodical way he killed, some taking months of tortue (sloth) or extreme violence (lust). The worst killers are the ones who think they are delivering a message and John Doe was delivering the message of sin. Hannibal Lecter was going to be my answer until I did a comparison. I couldn't answer any of the goofy horror monsters, they represent evil. Hannibal and John Doe are real, living evil.

Most psychotic killer? I think hannibal lecter tops my mental charts. His type of desire to kill is the most dangerous, no rhyme or reason & his supreme "ahead of the game" intelligence is just pure evil. Just a will to hurt/torture others, and eat them. So gross - it's very mind boggling to me, but that's the movies, but to be honest, I don't watch the scary ones often.

The guy in SAW is the most sadistic evil killer. You all should stop watching this trash, it will F**K you up for life.

Peter Lorre in the movie M...it wasn't how he killed the people that was creepy (because they didn't show it) it was the character. Very very creepy.
