Well, if you've washed behind your ears, then you've usually done the rest of your face. Thats the main purpose, to wash your face.

My mother used to say if i didn't wash behind my ears then I would grow cabbages there! So that is MY purpose for washing . I don't want cabbages!

As well as the hygiene considerations already described, I think there is a hint of 'do the job properly' in this adage. In other words, include the more difficult parts when washing as well as the easy ones.
