If your employer provided the phone and pays the bill each month, then it belongs to them and they can definitely monitor usage. Same goes with your PC and anything else your employer provides....Your time at work is not your own.

Not legaly if the Sim card is yours but if it is a company owned sim and phone they can to a degree but they must disclose the fact they are doing it, however there is software available for a price that can allow them to do this through illegal means

Not quite. Under the new Patriot Act your employer may screen, and listen to your calls, even if on your personal mobile device, while you are "at work". However, the employer cannot listen to your calls when you aren't "at work".

You dont say what country you are in.If you are in the UK yes only if your employer pays for and provides your phone and it is in their name.They can also tap your home phone if they provide it and are shown as the subscriber.Otherwise it is illegal.
