Different municipalities have different regulations and codes. Some don't even have a head-covering restriction for food service workers. Most that I have heard of do require it for those in food processing areas such as meat houses, packing plants, butcher shops, and bakeries, but restaurants are different. While it has long been recognized that hair nets keep most hair out of food, the danger from hair is greatest at the processing stage, not the cooking and serving stage. Most food service establishments require short hair, or that hair longer than shoulder length be restrained and pulled back. Furthermore, the danger of disease or harmful contamination spread from hair in a restaurant is far less than the chances of cross-contamination from prep surfaces, utensils, and storage practices and than the chances of food-borne diseases being spread by lack of proper hand washing and other disease conscious measures. What it boils down to is this: hair is undesirable because no one wants to floss their teeth when biting into a burger. The occasional hair won't typically carry disease so this becomes a customer service issue more than a health issue.
Besides, my wife finds hair in her food all the time, but it's usually hers! I have worked in places where there is virtually no chance of hair contamination, but customers still find hair. It either got into their food prior to reaching our establishment, by deliberate placement, freak occurrence, or it belongs to them or someone in their party.

Because the standard is that hair should be cut short or long hair is pulled back behind the back. I worked in a fast food restaurant and no one complained about hair in their food

They ought to. Because there has been plenty of times I got a hair in my food. And for some reason, Captain D's was the worst about hair being in the food. GROSS!!!!!!!!!!!
