My six year old son has not seen his mother since august. She informed me, at the conclusion of her mid-August visitation (every other weekend visitation), that she had been evicted from her apartment and would be moving elsewhere. She also stated that she did not know where or when she would be moving. I have not heard from her since. Indiana state law, i believe, requires that any parent , whether custodial or granted visitation, must notify the other if he/she relocates.My child has suffered developentally, behaviorally and emotionally from inconsistent visitation from his mother for years. There is no reason, other that the mental welfare of my son, that I would not want him seeing his own mother. I believe it is in the best interest of my son to legally deprive her of her rights to him, but I do not know if there is sufficient prove of reason for doing so (besides 8 months of noncommunication with her). Are there legal documents/websites I can review? Any help will be appreciated

I would look for a child custody lawyer. You can usually get an appointment for about 50 bucks just to see what they tell you can be done. They can start you on the right path to have sole custody awarded to you. Since the mother has had no contact with you or the son in 7 months, if you are still living at the same address, have the same phone number and work at the same place (so she can't say that you have made it impossible for her to find you), then you should not have a problem.

You of course, should consult a lawyer about this, but here goes...
Have you looked into abandonment?
Yes, She does have to send you information about her location - if you have not heard from her in 8 months that is a serious problem.
IN.Gov has a lot of useful info about custody and divorce rules... Hope I helped.
