I belive that they only country that still has an emperor is Japan with Emperor Akihito. However, many countries still have a monarchy of at least some form or another. Many of these countried were at one time Empires even though their rulers never claimed such a title, most notably England who were always Kings or Queens as their main title. Also, you could argue that even though Akihito claims the title, Japan really isnt an empire any more, so he is not really worthy of so lofty a title.
