Being a tennessee grad and a UConn grad student, I will second the notion that Lady Vols vs Lady Huskies is heated. My car got paintballed with blue and white paintballs shortly after I moved up here and started going to classes.
Aside from that....and in no order whatsoever really...here are 4 more I like.
1) Duke and UNC. Biggest because of the media hype surrounding it, the national presence of both programs, and the big names that always seem to be involved. This game almost always means something.
2) Tennessee and Memphis. This one isn't as well known nationally. But there is some hate there. There was a stretch for about ten years where each game was decided by 4 or fewer points and each game was hotly contested. Given the comments Calipari made about UT being a nice little "regional" program, followed by the beating the Vols laid on Memphis, I think it's only going to get hotter.
3) Xavier and Cincy- Definitely gotta agree here. Those two teams define hatred in a rivalry.
4) Kentucky and Louisville. How can you not say this in-state rivalry isn't hot? Pitino dumped some napalm on the flames when he took over at Louisville!

These are in no particular order:
1. Duke v. UNC
2. Indiana v. Kentucky (used to be a huge one)
3. Syracuse v. Georgetown (big one back in the day, starting to come back.)
4. Kentucky v. Louisville
5. Duke v. Maryland
6. Indiana v. Purdue (they trash each others campuses before this one)
7. Xavier v. Cincinnati
8. Syracuse v. UConn
9. Texas v. Oklahoma
10. Oklahoma v. OK. St.
The last two are more football rivalries that have spilled over to basketball.
There are so many more. If you go to the West Coast you'll hear about all the ones out there. I'm from the Mid-West so these are the ones I'm truly knowledgable of the history.

nothing comes before unc/duke. they are only miles apart and have been top notch programs for a long time. them playing each other 2-3 times a year makes it more intense.
here are mine....
1. duke vs. unc (second to none)
2. kentucky vs. louisville (this state is all b-ball)
3. Texas vs. Texas A&M (pure hatred, and some awesome games recently)
4. Indiana vs. Purdue (very intense, historic rivalry in the basketball state)
5. Duke vs. Maryland (second biggest in the ACC)
6. Syracuse vs. UCONN (historically the best of the big east)
7. Kentucky vs. Tennessee (very old rivalry, and now tennessee is getting good again)
8. Xavier vs. Cincinatti (couldnt rank this higher because the caliber of this game is lower, although it is a sick rivalry)
9. Kansas vs. Kansas State (throwing live chickens onto the court mocking Kansas!)
10. Kentucky vs. Florida (always last regular season game of the year)
Just missed my list: UCLA vs. USC (not just in football!), Kentucky vs. Indiana (they dont play each other much anymore so it isnt consistent), Florida vs. Tennessee (getting bigger every year), Kansas vs. Missouri, Vandy vs. Tennessee (neither team consistent over the years), WVU vs. Pitt (another football rivalry, but still a nasty one), Michigan vs. Michigan State, Ohio State vs. Wisconsin (a fairly new one, but will be huge for years to come).
The rivalries in the pac-10 dont seem as big to me (probably because i live on the east, haha)........but what is the biggest in the pac-10?
the bottom line is duke and unc is second to none because of the caliber of the teams, the history, and the location.

1. UK-Louisville (Kentucky fans actually came to Rupp arena during the tourney just to root against Louisville.)
2. Duke-UNC (it's second because they play 2-3 times every season.)
3. UC-Xavier (they are about 5 miles apart).
4. UCLA-Notre Dame (old school)
5. IU-Purdue (best I could come up with.

**Up and coming: UCLA vs USC will be good over the next couple of years esp. with Kevin Love and OJ Mayo going to the respective schools next year and the teams recent success.
