There are several ways to answer your question. It's true, the Emperor of Japan is the only sitting Royal still titled as "Emperor." However, Japan is not an empire. Furthermore, there are a number of other "imperial majesties" that can legally use the title, despite their lack of a throne. Any person who is the legal heir of a throne that no longer exists is called a "pretender." This is not the same as a person who pretends to be something that they are not (i.e. a fraud), since that person has no legal claim.
Charles Napoleon Bonaparte, Prince Imperial, is the current head of the Imperial House of France. His great-great-great uncle was Emperor Napoleon I. Charles Napoleon is a French politician and is running for the French Presidency under the Republican ticket. Otto von Habsburg is the pretender to the Austrian imperial throne, which was never technically an empire, even when it was the "Holy Roman Empire."
Now comes the matter of historical precedence. What constitutes an empire? Generally, an empire is a state that extends dominion over areas and populations distinct culturally and ethnically from the culture/ethnicity at the center of power. Technically speaking, this would make the United States an empire; certainly an economic empire. It would also make China an empire. In both cases, neither nation has an emperor. Despite the lack of popularity of the President, he will be out of office in two years. This fact directly contradicts any argument that he is an emperor. The same was true of the Roman Empire prior to the ascendancy of Augustus Caesar. Julius Caesar's highest position was "dictator for life," and he was never an emperor.

The only world leader that currently has the title of Emperor is Akihito of Japan. It's just a title, though - his actual power is minimal and significantly less than other world monarchs with "lesser" titles.
