Someone who is well-versed in a wide array of disciplines, inclusive of culture, art and history could be termed a Renaissance man. However, I do not know what he could be called if he had additional attributes like being physically fit, mentally ill and intuitive. Perhaps a Renaissance man, tormented by his sheer physical and intellectual excellence? Nietzsche succumbed to mental illness, as did ancient Greek philosoper Nero, along with a platitude of others.

Sounds like a talented individual who has a mental illness. Mental illness covers a wide range. Maybe he's a sociopath, bipolar, a schizophrenic, autistic, depressed, obsessive-compulsive, narcissistic, a kleptomaniac, a hypochondriac, or a psychopath. It's like asking how you classify a person with all of the above but who has type II diabetes.

almost sounds like a savant. that is a term used to describe an autistic person who has extraordinary focus on various subjects. for instance a savant in math could calculate complex or lengthy math problems in thier heads in seconds.
in the case of mental illness you wil have to be more specific about it's nature to get a more accurate description

dude, i'm all that plus financially successful and spiritually blessed as well!
intelligence? well, let's just say i have initals in front of my name.
i thought about hollywood, music etc but after a career in modeling...i hated being in the public's eye.
artistic? yep! but this method of earning a living is tough and you usually have to be emotionally challenged.
athletic? yep! competed at a very high level near national level. why did i not pursue it? this is a physically hard way to make a living.
so why do i like coming here on answers? because like so many things...i find interest in almost everything...for a while...
oh yeah, i'm mentally ill too!

How do you know he's mentally ill? He might just be someone gifted. Most of the times, people tend to classify other people based on their own criteria or perception of "normal". But really, normal is a relative term.
I'm saying this because your question (s) is a bit absurd. How can a mentally-ill person per se, be "gifted"? I thought mentally-ill people are those whose mental capacities are not at par to others of their age, gender, etc. Gifted people can be those with superior IQ or those exceptionally adept at a specifc field, e.g. physics, music, etc.
