The differences result from blood travel and distance from the main heat producing processes. Basically, the more energy used, the more heat generated. Muscles which are being used heat up. Relaxing lets them cool some. Metabolizing chemicals heats up the liver. It's all in a delicate balance. If the balance is lost, then the body is in shock. Extremities tend to be cooler. The nose and ears are very cold when compared with the rest of the body. They don't do much physically or chemically. Their structure is functional so they don't have to metabolize or use energy to do anything more than stay alive.

Different organs do have different temperatures but they are all pretty close. The skin (it is an organ) is colder than your heart most of the time. Blood cools as it moves out into the body so extremities are cooler.
