Many fans suggest it is Marcellus Wallace's soul.
The bandaid on the back of his neck suggests that is the means by which it was extracted. Actually, the bandaid was put there to cover up a scar that Ving Rhames has, but that's another story.
Some even think it might be the Holy Grail.
It's left ambiguous, so guys like you and I talk about it non-stop.

Quentin Tarantino said that mainly it is supposed to be Marcellus' soul. That's why you see the band-aid on the back of his neck. It's on the special features of dvd.
